DEATH CERTIFICATES

PLEASE NOTE:

  • Application forms must be completed in black ink ONLY.
  • E – Mail address must be provided for every application that is made. To ensure that the address provided is as clear as possible, please, write legibly.
  • Payments must be in British Pounds in the form of money order, bank draft or cash.
  • Postal orders must be made payable to SA High Commission.
  • Special delivery envelope must be provided for all applications The said envelope will be used to return all issued certificates to the applicant securely. Certificates issued to applicants who sent their applications to this office without special delivery envelope will not be posted.
  • The person making the application MUST state clearly the reason for the application.
  • The type of death certificate being applied for MUST be clearly made. The following types of death certificates are issued: UNABRIDGED, COMPUTER PRINTED, CERTIFIED COPY OF DEATH REGISTER OR HAND WRITTEN ABRIDGED DEATH CERTIFICATE.
  • If you have a previous certificate, PLEASE, attach a certified copy so that it may be easier to trace the record.

 

WHO MAY APPLY AND UNDER WHAT CONDITIONS CAN AN APPLICATION BE MADE

Any person may apply for a copy of a Death Certificate provided the particulars have been registered with the South African Government. The fact that the person concerned may have died in South Africa does not necessarily mean that his/her death was registered with the South African Government.
